Do have a script or particular service you're using to run the newsletter? We can't really give you guidance without knowing what you're using.

He probably wants the HTML version of the form... Which I am almost surtain of seeing that we talk every day about his/our site(s).....
But I could help him but the HTML version of the form doesnt work for me nor him. It just opens an email-panel with only the email-adress in it, instead of sending the info directly to the webmaster's emailadress.

Not really an idiots guige and to be honest trying to tell an idiot how to make there own form is pretty hard thing to do. If you want it hosted at another site, probably the easiest and quickest way of doing it, then i suggest looking at http://www.compactvision.co.uk/index.html . That was the first ever form that i used, not only is it set up for you, all you have to do is copy the code onto your website and thats it. Everything is managed on their website by you.
http://php.resourceindex.com/Complete_Scripts/Mailing_Lists/
If your server allows the use of PHP.
and
http://www.hotscripts.com/
To search for other PHP and HTML scripts, i presume they will also have a section dedicated to mailing lists in most of the sections if not all of them.
Even though you asked how to create a newsletter registration form, most of these create that for you especially the compactvision one, which you can fit into your site. Asking just to create the form would be pretty stupid, without a script to quicken the sending and managing process up.
If you need any more help just send me a message, replying to this message could take me a couple of days to respond too....dont check it out too regularly. Hopefully my answers not too late as i cant remember checking the post date.
P.S. good idea for a thread.

best is to use php, you created a database SQL where the mail of the members is recorded and you send all with function mail()
Well my explanation should not be very clear, I am French...
